Celebrity Plastic Surgeon Was Gunned Down Inside His Clinic After Warning Police About Threats Months Earlier

A renowned plastic surgeon who treated celebrities and international clients had reportedly warned police that he was receiving threats months before he was shot dead inside his own clinic in southern Spain. Dr. Alexander Aslani, 57, was killed in a shocking attack that has left the medical community stunned and raised disturbing questions about whether his murder was planned in advance.

The fatal shooting took place at around 6 p.m. on Wednesday, October 7, at Cirumed Clinic in Marbella, where Aslani was working when a masked gunman entered the building. According to reports cited by PEOPLE, the attacker shot the surgeon at close range before fleeing. Emergency medical personnel arrived and attempted to save him, but Aslani later died from his injuries. The married father of two had spent decades building an international reputation in cosmetic surgery.

Now, investigators are examining a troubling detail that emerged after his death: Aslani had reportedly filed a formal complaint with police in April, saying he had received threats connected to his professional work. Spanish newspaper Sur reported that detectives are reviewing previous disputes, medical claims and complaints involving dissatisfied patients in an effort to determine whether any of them could be connected to the killing.

The investigation has also focused on a series of alleged threats made shortly before the shooting. According to reports cited by PEOPLE, a patient contacted the clinic on the day Aslani was killed, expressing anger over the results of a cosmetic procedure and demanding an immediate refund. The messages allegedly included explicit threats. Investigators are also examining reports that the patient’s partner visited the clinic during the previous week and issued warnings to staff.

Authorities are exploring whether the killing could have been a contract attack arranged by someone with a grievance against the surgeon. An 18-year-old Swedish national was arrested in Málaga on October 8 in connection with the fatal shooting. Police are continuing to investigate the suspect’s possible role and whether anyone else may have been involved. However, investigators have not publicly established a definitive motive, and the alleged connection to a dissatisfied patient remains under investigation.

Aslani was widely known in the cosmetic surgery world for his work on Brazilian Butt Lift procedures, including a technique promoted by his clinic as the “Super-Charged BBL.” His practice attracted patients from more than 40 countries, with a clientele that reportedly included celebrities, models and members of royal families. He had also treated several British reality television personalities, helping establish his profile beyond the medical industry.

Following news of his death, tributes began appearing from colleagues and former clients. The European Society of Aesthetic Plastic Surgery praised his professional legacy, noting his more than three decades of experience and contributions to body-contouring surgery. Former clients also shared emotional messages online, remembering the doctor as a friend as well as a medical professional.

For investigators, the central questions remain painfully urgent: Who wanted Aslani dead, and were the threats he reported months earlier connected to the attack? Detectives are now working to reconstruct the events leading up to the shooting and determine whether the warning signs could help explain what happened. Until more evidence emerges, the motive behind the killing remains unconfirmed.
